Kevin Ropp, director of the OSHA office of communications told Hoist: “I can confirm that OSHA’s Toledo, Ohio, Area Office has initiated a fatality investigation.”
Jose Diaz, 35, was understood to be performing contract work for Superior Environmental Solutions inside the mill’s basic oxygen process shop.
According to a report in Ohio-based publication the Chronicle-Telegram: “Police said Diaz had climbed a ladder to clean an area near a crane. When he reached the third and final step of the ladder, he went to step down onto a small ledge and either slipped or tripped on a steel rail, losing his balance.”
